Welcome to the Your Health-Key (“YHK”) Web site (the “Site”). YHK provides online health-related services by arranging for telephone, chat, email or video medical consultations with a licensed doctor, arranging for electronic medical record storage by the doctor, providing certain on-line health-related information, and offering certain other health-related services, which include online booking appointments and storing your health reports securely. As used in these Terms of Use & Privacy Policy, references to “YHK” means Your Health-Key. As used in these Terms of Use & Privacy Policy “you” or “your” means you, your dependent(s), if any, and any other person accessing your YHK account. These Terms of Use & Privacy Policy apply to your use of the Site and, to the extent referenced, to your registration for, subscription to, and use of YHK Services. 

 

Declaration: 

• You declare that You wish to take part in the Your Health-Key Doctor’s telephone, and online medical consultation services. These services are provided to You through telephone, chat, email, and video consultations form given on our website by our fully registered Indian medical practitioner. All such online consultations and treatment suggested are subject to the Terms of Use & Privacy Policy set out in this agreement.

 

• Your Health-Key cannot guarantee that a doctor consultation made through this Website will result in a prescription being issued by the prescriber.

 

• I give my consent for information about myself, my health and my current medications including, but not limited to, the information I divulge as part of the online consultation forms to be viewed and exchanged by and between Your Health-Key and its Doctors working for Your Health-Key for the purpose of conducting a medical consultation. I understand this information may be exchanged electronically.​

 

• You warrant and undertake that all information provided by you in our online consultation forms given on our website is correct and true and that you have not withheld information that you believe may be relevant or useful in any way.

 

• All medicines prescribed will be based on the consultation form completed by you and telephonic or chat or email or video conversation between you and our doctor. Therefore, we and our doctor take no liability for loss or damage arising from our service or medication prescribed if you supply incorrect or incomplete information.

 

• Your Health-Key and their respective directors or associated doctors shall not be liable for any losses or claims arising directly or indirectly from use of this Website or its services or any products supplied except that this exclusion of liability does not apply to any damages in connection with death or personal injury caused by the negligence of Your Health-Key or their respective directors or associated doctors.

 

• All content of this website aims to facilitate professional medical advice. It is designed to support, not replace, the relationship between you and your healthcare providers.

• Your Health-Key, the creator of this Privacy Policy ensures its commitment to Your privacy with regard to the protection of your information. In order to provide You with uninterrupted use of our services, We may collect and, in some circumstances with your consent, disclose information about you. Such information may be classified as personal information under the purview of Regulation 4 of the Information Technology (Reasonable security practices and procedures and sensitive personal data or information) Rules, 2011. • In order to use the services of this Application, You are required to register Yourself by verifying the authorised device. Please read the following Privacy Policy to understand how your personal information will be treated as you use this Site and its services. The following discloses our information gathering and dissemination practices.  We may collect the following information:

       a. Personal data of the User such as, but not limited to, Your name, Your age, date of birth, height, weight, and gender;

       b. The User’s e-mail and contact information;

       c. The User’s tracking Information such as, but not limited to the device ID;

       d. The User’s data sent across through the Application.

 

• As a User of YHK, you may provide information about yourself, your spouse or family, your friends, their health issues, gender of the patient, age of the patient, previous medication taken, previous medical conditions, allergies, etc.

 

• The Information specified above and collected by us may be classified as ‘Personal Information’ or ‘Sensitive Information’ under the Information Technology (Reasonable security practices and procedures and sensitive personal data or information) Rules, 2011. Collection of information which has been designated as ‘sensitive personal data or information’ under the Information Technology (Reasonable security practices and procedures and sensitive personal data or information) Rules requires your express consent. By affirming your assent to this Privacy Policy, you provide your consent to such collection as required under applicable law. Our Services may be unavailable to you in the event such consent is not given.

 

• For Practitioners using YHK, we would collect your Name, Education, Specialization, address of practicing hospitals/clinics, Contact information, achievements, Device ID (IMEI), Location, etc. If you use YHK by accessing it through your Google+ and Facebook Profiles and other API’s, we collect such data as well.

 

• The Application may include offers from our vendors, advertisements, hyperlinks to other websites, applications, content or resources. We have no control over such external links present in the Application, which are provided by persons or companies other than Us.

 

• You acknowledge and agree that We are not responsible for any collection or disclosure of your Personal Information by any external sites, applications, companies or persons, nor are do We endorse any advertising, products or other material on or available from such external application, websites or resources. You further acknowledge and agree that We are not liable for any loss or damage which may be incurred by You as a result of the collection and/or disclosure of Your personal information by external applications, sites or resources, or as a result of any reliance placed by You on the completeness, accuracy or existence of any advertising, products or other materials on, or available from such applications, websites or resources. These external applications, websites and resource providers may have their own privacy policies governing the collection, storage, retention and disclosure of Your Personal Information that You may be subject to. We recommend that You enter the external application or website and review their privacy policy.

 

• We may allow third parties/individuals to display advertisements when you use the Application.

• The information made available by Patients is displayed to Practitioners and vice versa. The information collected from Practitioners is displayed on the Application to enable a User to connect with a Practitioner of their choice.

 

• Any information that you submit to us via our Website - whether through the basic information form about you and your health problems, personal history form, or e-mail will be used exclusively by the consulting doctors and only for the specific reason for which they were submitted. Your Health-Key is the sole owner of the information collected on this site. We will not sell, share or lease this information to others. Your Health-Key does not sell customer lists, e-mail addresses, or any other your personal data/information. By using this site and the Your Health-Key services, you signify your acceptance of this Privacy Policy, as stated.

 

• YHK may update this privacy policy from time to time at YHK’s sole discretion.

Refunds:

• YHK may provide refunds as determined in its sole and absolute discretion.
